Our goal is to meet each resident’s needs while maintaining the highest level of choice, dignity, privacy, respect and individuality. To assure this is done effectively, an assessment is conducted prior to move-in. Residents and/or family members work together with Heritage Place staff to develop an individually tailored care plan. Follow-up care conferences are held regularly to assure that as a resident’s need change; the care plan is adjusted accordingly.
